School District Responsibilities to the Evaluation Process

  • Issue Permission to Evaluate form and Procedural Safeguards Notice.
  • Send copy of Permission to Evaluate to psychologist if needed a Referral for Support Services to the Indermediate Unit 1 Program Supervisor.
  • If parent agrees to share educational records, notify nonpublic school principal/director that a child enrolled in his/her school is being evaluated for special education and related services. Forward copy of Permission to Evaluate using Notification of Nonpublic Evaluation Form (Word Document) to nonpublic school and IU1 Consultant for Nonpublic Schools along with the projected 60th school day of the process.
  • Psychologist contacts nonpublic school principal/director to arrange a date to test the child in the nonpublic school or other site as agreed to as the parent.
  • Contact IU1 Consultant for Nonpublic Schools to obtain pre-referral information from nonpublic school personnel.
  • Obtain Evaluation Report from the psychologist.
  • Conduct Evaluation Report (ER) meeting (invite nonpublic school and IU1 Consultant for Nonpublic School).

    If parents decline Free Appropriate Public Education (FAPE), re-evaluate every three years (two for a child with Mental Retardation).

    If parents wish to consider dual enrollment or FAPE:

    • Schedule and conduct Individualized Education Program (IEP) team meeting. The following options are discussed by the team:
      1. Parent accepts FAPE — issue Notice of Recommended Educational Placement (NOREP)
        or
      2. Parent accepts dual enrollment (Not FAPE — No IEP goals) Issue NOREP
        or
      3. Parent refuses FAPE via the NOREP
